I honestly forgot about my wisdom teeth and I signed my Army Contract!!!?

I know there are going to be some haters out there, but i recently signed my army contract. Now, understand that I am human and I am going to make many mistakes in my life, but i honestly forgot to put that I had wisdom teeth out on my medical part of the contract. The reason I forgot was that I just kept thinking of it as a typical dentist appointment, the surgery went to smoothly and i honestly forgot i had them out. That same day I was out and about doing things, I never spit blood, I never took any of the medications, I just honestly forgot. And now here I am, months away from shipping to BCT and I need to find a way to get this mess cleared up. It was an honest mistake and I am kicking myself for forgetting about it. HELP PLEASE!?!
